Changing Providers

If you are currently receiving care and looking for a new provider, changing can be a big step.

 

At Carehero, we recognise the importance of this and have developed a structured system to ensure your transition is seamless, transparent, and safe.

 

At the heart of the transition is your feedback and preferences.

 

Whether you choose to keep your current team or select a new care team (or a combination of both) we have the expertise to accommodate a successful transition.

 

Timeframes can vary depending on the circumstances unique to you, however, the process can be safely implemented between 2 to 5 weeks.​​

Phase One

 

  • What’s important to you: Getting educated on your clinical and social needs.

  • Your Current Roster: Analysing your current roster and address any gaps.

  • Care Plan: Developed in consultation with you and our Clinical Coordinator.

  • Current Team: Identify staff you would like to transition.

  • Set Start Date: Agree on a future date to work towards.

Phase Two

  • Recruitment: Enter job boards and explore current database.

  • Staff Selection: You decide who walks in and out of your home.

  • Meet & Greets: Informal catch-up arranged.

  • Staff Transition: Current staff register with Carehero.

  • Develop Induction: Specific to you.
